Health Benefits


Medical Insurance

Eligibility: Regular employees who work at least 30 hours per week.

Coverage effective date: Coverage is effective the first of the month following 30 days of eligible status.

Description: Triple Gold Health Care Plan administered by BCBS requires you to choose a Primary Care Clinic at time of enrollment.  Plan provides an 80% benefit for most in-network services after annual deductible is met.  Extended Network benefits provided after deductible and higher co-insurance.  www.bluecrossmn.com


Dental Insurance

Eligibility: Regular employees who work at least 30 hours per week.

Coverage effective date: Coverage is effective the first of the month following 30 days of eligible status.

Description: Choice between preventive only coverage and preventive/restorative coverage.

Flexible Spending Accounts - Health Care Reimbursement Account

Eligibility: Regular employees who work at least 20 hours per week.

Coverage Effective Date: Coverage is effective the first of the month following 30 days of eligible status.

Description: Allows you to establish a personal spending account with before-tax dollars to pay for out-of-pocket medical and dental expenses.  You may set aside up to $3,500 per calendar year.



Flexible Spending Accounts - Dependent Care Reimbursement Account

Eligibility: Regular employees who work at least 20 hours per week.

Coverage Effective Date: Coverage is effective the first of the month following 30 days of eligible status.

Description: Allows you to establish a personal spending account with before-tax dollars to pay for childcare expenses that are necessary to allow you to work.  You may set aside up to $5,000 per calendar year ($2,500 if you are married and filing separate Federal income tax return).


Life Insurance


Basic Term Life Insurance

Eligibility: Regular employees at least 18 years old, who work at least 20 hours per week.

Coverage effective date: Coverage is effective the first of the month following 30 days of eligible status.

Description: Provides your designated beneficiary with two times your base pay in the event of your death while employed at Courage Center.


Voluntary Life Insurance

Eligibility: Regular employees at least 18 years old, who work at least 20 hours per week.

Coverage effective date: Coverage is effective the first of the month following 30 days of eligible status.

Description: Employee coverage:  Provides your designated beneficiary with the amount of money you select in the event of your death.  This benefit is in addition to your basic term life insurance.  You may purchase insurance in $10,000 units with a life maximum of the lesser of $500,000 or five times your basic annual earnings. 

Dependent coverage: You also have an opportunity to purchase life insurance for your spouse/domestic partner and/or children.


Accidental Death & Dismemberment

Eligibility: Regular employees at least 18 years old, who work at least 20 hours per week.

Coverage effective date: Coverage is effective the first of the month following 30 days of eligible status.

Description: Provides up to two times your base pay in the event of an accidental death or dismemberment. 

Voluntary Accidental Death & Dismemberment

Eligibility: Regular employees at least 18 years old, who work at least 20 hours per week.

Coverage effective date: The first of the month following 30 days of eligible status.

Description: Provides up to two times your base pay in the event of accidental death or dismemberment.


Disability/Salary Continuation


Short-Term Disability

Eligibility: Regular employees at least 18 years old, who work at least 20 hours per week.

Coverage effective date/Description: This benefit replaces 50 percent of your monthly pay starting the first of the month following one year of employment with Courage, and 75 percent of monthly pay starting the first of the month following two or more years. Provides income continuation in the event of a personal illness or injury which prevents you from working.  Income replacement begins on the sixth consecutive day of your illness, accident or injury for which a doctor determines you are unable to work, but for no longer than 90 days.


Long-Term Disability

Eligibility: Regular employees at least 18 years old, who work at least 20 hours per week.

Coverage effective date: Coverage is effective the first of the month following 30 days of eligible status.

Description: Provides income continuation in the event of a personal illness or injury which prevents you from working for more than 90 days.  This benefit is equivalent to 60 percent of your earnings reduced by 100 percent of Social Security Disability benefits and/or worker’s compensation.


Time Off


Paid Time Off (PTO) - (includes vacation, sick, holiday and personal time)

Eligibility: Regular employees who work at least 20 hours per week. 

Description: Paid time off for vacation, sick leave, holidays and personal time.  One accrual based on hours worked each pay period.


Retirement

403(b) Plan

Description: A retirement savings program with deferred federal and state taxes on deposits and interest.  http://www.mutualofamerica.com/

Eligibility for Employer Match: All employees.

Eligibility for Base Contribution: Employees working at least 1,000 hours in calendar year and employed on Dec. 31.
