What the Neuro Care Navigator Does
Courage Center’s Neuro Navigator acts as an internal case manager to outpatient clients with a diagnosis of traumatic brain injury, stroke or acquired brain injury, who use two or more medical rehabilitation services at our Golden Valley facility.
Reminiscent of the plate spinners of old, the navigator works with multiple people to provide a comprehensive treatment approach.
The navigator promotes and facilitates communication among the client, the family and the therapy team at Courage Center, as well as social workers, physicians, insurance companies and other professionals who are part of the client’s care team. The navigator is the first person the client and family encounter, and an ongoing presence and resource during services at Courage Center.
Navigation services include initial contact, scheduling, family conferences and ongoing coordination of care with the client, physician and neuro comprehensive team. This unique service gives Courage Center client’s a go-to person to assist with complex needs, ensuring quality outcomes and client satisfaction.
The Neuro Comprehensive Teams
The teams include a primary occupational therapist, physical therapist, speech- language pathologist, social worker and the navigator. Many members of the neuro teams are Certified Brain Injury Specialists (CBIS) through the Brain Injury Association of America.
The goal of the Comprehensive team is to have one primary therapist in each discipline for continuity of care. The teams meet on a weekly basis to facilitate communication and a comprehensive plan of care. Client’s cases are discussed as needed and/or every 30 days. During these discussions the team may identify the need for additional services and assist the client in obtaining them.
All Courage Center navigators have had previous clinical experience in direct patient care. This background allows them to bring clinical expertise to navigation, and is key in determining appropriate services and education for the client, family and care team.
Outcomes Measurement: The Mayo Portland Adaptability Inventory is used to track clinical outcomes.
