Our Unique Outlook on Primary Medical Care

Courage Center’s Primary Care Clinic, a health care home, is a new way to experience primary medical care. We offer medical, rehabilitation and psychiatric care in one clinic location. Our doctors, nurses and nurse practitioners understand chronic medical problems. They are experienced in coordinating all aspects of care and services, while keeping the process easy and uncomplicated.  Our goal is to partner with you to keep you as healthy and independent as possible. Since the initiation of our Health Care Home, we have reduced inpatient hospitalization stays for our clients stays by 76 percent. (See our Outcomes page for details.)

Courage Center's Clinic provides:    

• Comprehensive health care services, including medical, rehabilitation and psychiatric services
• Improved coordination with sub-specialists; coordination of services provided by medical and community resources
• Preventive and wellness care
• Pain management
• Ongoing nurse support and follow-up                 

In joining our Health Care Home, you are responsible for:

• Being an active partner with your care team
• Learning new strategies for managing your helath
• Following your care plan
• Giving us feedback
