Courage Center's health care home clinic for people with disabilities and/or complex medical conditions offers coordinated medical, rehabilitation and psychiatric care at the Physicians' Clinic in our Golden Valley facility.
Since the initiation of our health care home, we have reduced inpatient hospitalization stays for our clients by more than 75 percent. In May 2012, the Centers for Medicare and Medicaid Services (CMS) selected Courage Center as one of 26 nationwide recipients of the Health Care Innovation Award. The three-year $1.8 million award is allowing us to further develop our patient-centered medical home model.
